The Jeanne M. Lusher Center of Hemostasis and Thrombosis is happy to offer the following clinical and social services to meet the needs of the patients we care for:
Social Support Services

Assistance with navigating barriers to receiving medical care including assistance with transportation to clinic not covered by insurance

Providing education on how to utilize transportation through medical insurance (if applicable)

Psychosocial assessments to assess factors that impact the overall needs of the patient and family, including mental health support and resources

Providing resources to address needs that are not being met, as determined by the psychosocial assessment

Assistance with financial needs through community support services and navigating support through Michigan Department of Health and Human Services (MDHHS)

Coordination of educational opportunities outside of clinic including:

Educational dinner events

Events hosted through regional or national programs

Primary facilitator of the patient advisory committee

Evaluation of barriers and challenges for adherence to medical plan of care

Working with patients and families to find a patient centered solution to improve compliance with prescribed treatments, medications, and attendance of appointments

Assistance for adolescents and young adults in learning to be independent in managing their bleeding or clotting disorder to better prepare for the transition from pediatric to adult care

Home Treatment Education Program
Patients/patient families are taught how to provide factor replacement through venous access. Over several weeks, both written and hands on education is provided from nursing staff to assure that the patient/patient family is prepared and comfortable to infuse factor treatments into a vein. This process promotes independence and allows the family to have the freedom to treat away from the treatment center.
